What is the Medicare GLP-1 Bridge?                                                                                                         

For many Medicare beneficiaries, access to newer GLP-1 medications has been limited by cost and Medicare coverage rules. That has changed with the launch of the Medicare GLP-1 Bridge, a new federal demonstration designed to make certain GLP-1 medications more accessible for eligible Medicare beneficiaries who use them for weight management. 

The short term program began July 1, 2026, and is currently scheduled to continue through December 31, 2027. Eligible beneficiaries can obtain certain GLP-1 medications for a $50 monthly copay, subject to the program's eligibility and prior authorization requirements.

The Bridge operates outside the normal Part D benefit and payment structure. CMS uses a central processor to handle prior authorization, claims processing, and pharmacy payments.

What Are the Potential Benefits?

1. A predictable $50 monthly copay

Because the medication is provided outside the normal Part D benefit, the Part D deductible does not apply to the Bridge medication, and the $50 copay does not count toward the beneficiary's Part D true out-of-pocket costs.

For people who have faced high out-of-pocket costs for GLP-1 medications, a predictable monthly amount may make treatment easier to budget for.

2. Access to certain GLP-1 medications

As of the current CMS guidance, medications available through the Medicare GLP-1 Bridge may include:

  • Foundayo®
  • Wegovy®, including injection and tablet formulations
  • Zepbound® KwikPen®

3. A potential option when traditional Part D coverage does not apply

Medicare coverage of GLP-1 medications depends heavily on the reason the medication is prescribed.

The Bridge is specifically designed for eligible beneficiaries seeking these medications for weight management when the use is not otherwise covered through their Part D benefit.

That distinction is important: the Medicare GLP-1 Bridge is not simply a replacement for Part D coverage.

Who May Qualify? Who Is Not Eligible Through the Bridge?

Eligibility is based on several factors, including Medicare Part D enrollment, the reason the medication is prescribed, and specific clinical criteria.

The first step is to speak with a healthcare provider about whether a GLP-1 medication is medically appropriate.

The provider can then determine whether the patient appears to meet the Bridge's clinical criteria and submit the required prior authorization request.

Medicare beneficiaries can also review information about the program through Medicare.gov and contact Medicare for questions about eligibility and participation. CMS directs beneficiaries to 1-800-MEDICARE (1-800-633-4227) for additional information.

The program has important limitations.

For example, beneficiaries who are using an eligible GLP-1 medication for certain conditions already coverable under Medicare Part D may need to obtain the medication through their Part D plan instead.

The Medicare GLP-1 Bridge is a new, time-limited CMS demonstration that may make certain GLP-1 medications more affordable for eligible Medicare Part D beneficiaries seeking weight-management treatment.
